6.) Perform data collection essential for carrying out the plan of care under the direction and supervision of a physical therapist (7-12)

The artifact that I decided to choose that fits this program outcome is the medication report that I did on a patient while in school. This fits because I performed some data collecting on the medication she was taking and how it was affecting her both good and bad. For example she was taking Baclofen for the muscle spasms in her lower back and she did say that she wasn't getting them as much as she used too. She also said that she feels pretty tired throughout the day and drowsiness is a common side effect of the drug. Overall, I looked up that five medications she was on and further educated her on the effects and side effect of each one.
